Description
Published in 1991, the Neck Disability Index (NDI) was the first instrument designed to assess self-rated disability in patients with neck pain.
Structure / Content
The patients choose the statement that best describes their situation in each of ten sections. The sections concern impairments like pain (including headaches), and abilities like personal care, lifting, reading, driving, and recreation. The queationnaire can be found on below.
Scoring Method
Same as the Oswestry Disability Index. Total scores can range from 0 (highest level of function) to 50 (lowest level of function), and "percentage of disability" scores are calculable.
Cost
None. The instrument is in the public domain, so clinicians can use it without obtaining permission.
